As the Philippine Statistics Authority (PSA) continue with the registration for the Philippine Identification System (PhilSys) and issuance and delivery of printable e-PhilIDs, several registration centers receive reports of lost and damaged Philippine Identification (PhilID) cards. Being the primary implementing agency, the PSA has the duty to ensure that the PhilID cards are durable and in good condition. Hence, the PSA will issue a replacement for PhilIDs with damage and defective front-facing photographs upon request of the concerned PhilSys registrant. Moreover, a replacement PhilID will also be issued for lost and damaged PhilID cards due to natural calamities, upon submission of the necessary requirements.

In view of the foregoing, the front-facing photograph of the PhilID card is considered damaged or defective in the following instances:

(a) the whole photo or part of it is peeled off;
(b) the photo is not similar to the ghost image; and
(c) the photo is washed out and the facial features are not recognizable.

Meanwhile, for lost and damaged PhilID cards due to natural calamities, a certification signed by the Punong Barangay, where the cardholder resides, shall be required. Furthermore, the damaged PhilID card must be surrendered upon application for investigation. Interested

PhilSys registrants may visit the nearest Philsys registration center in their locality to file their request for replacement, free of charge.
