Last 13 February 2024, PSA Bohol started the implementation of the Registration of Children ages 1-4 years old. Only the demographic information and front-facing photograph of the children should be collected during the PhilSys Registration. The complete biometric information of the children shall be initially captured once they reach the age of five.

The PhilSys Number (PSN) of the children shall be linked to the PSN of the parent or guardian for authentication purposes. To clarify, the children should be accompanied by their PhilSys registered parent or guardian during the registration process. Please refer to table 1 below for the documentary requirements for each classification.

For parents who wish to register their kids to PhilSys, present the original copy of any of the following documents:

  • Certificate of Live Birth issued by the PSA or Local Civil Registry Office (LCRO);
  • Report of Birth issued by the PSA or Philippine Foreign Service Post (PFSP);
  • Certificate of Foundling issued by the PSA;
  • Certificate of Foundling or Certificate of Live Birth of Person with No Known Parent/s issued by the PSA;
  • Municipal Form No. 102;
  • Philippine Passport or ePassport issued by the Department of Foreign Affairs (DFA);
  • Any document showing the (1) full name of the child, (2) date and place of birth, (3) the full name of the mother, and (4) the full name of the father (if acknowledged).

For those parents who would authorize someone to accompany the registration of their child, please be informed that if the PSN of the authorized person has been linked to the PSN of the child, it may no longer be unlinked in the event that either the parent wishes to link his/her PSN to that of his/her child.

Interested applicants can also register in the PhilSys Fixed Registration Center (FRC) located at 2F Galleria Luisa Building, Gallares St., Tagbilaran City. The center is open from Monday to Saturday, 9 AM to 6 PM.
